We do not share or sell personal information about you to any other person or entity. Personal information may be shared with employees or contract service providers of Body Therapy Services who we believe need to have access only to that information necessary in order to do their jobs.

1. Please complete and submit the Customer Contact Form if you are an existing customer updating your contact information or if you are a new customer.

2. Massage customers: please take a moment to print the Massage Intake Form, fill it out, and bring it with you on your first visit.

3. For skin care customers, there is no additional form for you to complete at this time.
